1. Change of Board of Directors

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copies of Form 24 and Form 49

ii. Sole Proprietorship / Partnership

Peninsular Malaysia

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copy of the Business Information Form (SSM)
• Certified copy of the Current Owner's Information (SSM)
• Certified copy of the SSM Certificate of Registration (Form D)

Sabah

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copy of Form B

Sarawak

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copy of the Registration Certificate and Business Name Registration Confirmation

iii. Organization / Association / Union / Cooperative / Institution

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copy of the Organization Registration and list of Committee Members

2. Change of Printing Premises Address

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copy of the Business/Premises License from the Local Authority (PBT)
• Certified copy of the Letter of Approval for Machinery Installation from the Department of Occupational Safety and Health (DOSH)
• A4-sized location map of the premises

Note: The printer is required to be registered with the City Hall/State Secretary's Office (SUK)/Local Authority (PBT) and DOSH. They must submit the premises/business license from PBT and the letter of approval for machinery installation from DOSH to the Ministry when applying.

3. Change of Company Name

• A completed Printing Press License Application Form from the company/business/organization/institution
• Certified copies of Form 9 or 13, Form 24, Form 32A, Form 49, and the Memorandum & Articles of Association (M&A)
